Wavier Program

Community Care Services Program (CCSP)

CCSP is a program of the Georgia Department of Community Health, which helps senior adults and people with functional and/or cognitive impairments stay in their homes and communities for as long as possible.

The CCSP service options include the following:

  • Adult Day Program

    • Available up to five days a week

    • Daytime care and supervision in an adult daycare center

    • Therapeutic activities and outings

    • Meals adapted to prescribed diets

    • Dementia-specific care

  • Alternative Living Services

    • Alternative residence for persons unable to remain independent in their homes

    • Meals, medically-oriented personal care and nursing supervision

  • Emergency Response Services

    • 24 hour, 7 day in-home electronic support between the client and a medical control center

    • Personal Emergency Response System

  • Home Healthcare Services

    • Skilled nursing services—physical, speech & occupational therapy, home health aide, medical social services and medication management

    • Medical case management

  • Respite Care

    • In-home & out of home (where available)

Service Options Using Resources in Community Environments (SOURCE)

Creates a continuing care network for at risk individuals who need medical attention, supportive assistance and prefer to stay in their own homes and communities. This alternative to nursing home placement provides comprehensive, preventative medical care along with in-home support services. Care management is central to identifying and providing appropriate service. SOURCE service options include the following:

 

Comprehensive Primary Medical Care

  • Specialist care coordinated through the participant’s primary care provider

  • Visiting Nurse Care Manager to coordinate care

 

Adult Day Programs

  • Daytime care and supervision in an adult daycare center

  • Available up to five days a week

  • Therapeutic activities and outings

  • Meals adapted to prescribed diets

  • Dementia-specific care

 

Personal Support Services

  • Assistance with meal preparation, hygiene and nutrition

  • Light housekeeping, shopping and other support services

 

Respite Care

  • In-home & out of home (where available)

 

Home Healthcare Services

  • Skilled nursing with medication management

  • Medical care management

 

Enhanced Case Management

  • All social and medical needs are supported by individual Care Managers

 

Alternative Living Services

  • Alternative residence for persons unable to remain independent in their homes

  • Meals, personal care and supervision

 

SOURCE Eligibility Requirements

  • Have SSI Medicaid or Public Law Medicaid coverage

  • Functional impairment caused by physical limitation

    • NOTE: Alzheimer’s and dementia are physical conditions

  • Unmet need for care

  • Approval of care plan by applicant/client’s physician

  • Services fall within the average annual cost of Medicaid reimbursed care provided in a nursing facility

  • Approval of an intermediate level of care (LOC) certification for nursing home placement

  • Client chooses community based rather than institutional services

  • Health and safety needs can be met by SOURCE

  • Participation in no other Medicaid waiver program

  • Medicare home health services do not meet their needs

  • The need for services beyond home delivered meals

  • A home environment free of illegal behavior and threats of bodily harm to other persons

In-Home Nursing Services for Medically Fragile Children Under the Age of 21
The Georgia Pediatric Program (GAPP) offers the opportunity of at-home nursing care and personal care to medically fragile children. Health Force of Georgia is a state-of-Georgia provider of professional, skilled nursing care to children in the comfort and safety of home.
 
GAPP In-Home Nursing Services
Skilled nursing and personal care services provided under GAPP include:

  • Assistance with daily living activities

  • ​Bathing

  • ​Meals

  • Housekeeping

  • Monitoring vital signs such as blood pressure, pulse, respiration & temperature

  • Assistance with ambulation & transfers

  • IV therapies

  • Wound care

  • Tube feedings

  • Gastrointestinal disorders


Is Your Child Eligible for Care Under GAPP?
GAPP is a Medicaid program for medically-qualified children under the age of 21. Children of families eligible for Medicaid can apply for GAPP at any time and must meet the same level of care necessary for admission to a hospital or skilled nursing facility. GAPP is managed by the Georgia Department of Community Health (DCH).

In-Home Support Services for Adult Medicaid Recipients in Georgia with Severe Physical Disabilities and/or Traumatic,Brain Injury (TBI)

The Independent Care Waiver Program (ICWP) in Georgia offers personal, in-home support services to help adult Medicaid recipients with severe physical disabilities live in their own homes or in the community, instead of an institution such as a nursing home or hospital.

 

Health Force of Georgia is a Georgia-approved participating provider of at-home care services to adults eligible to receive ICWP. This means persons who qualify for ICWP may receive homecare services at no cost to them because Health Force of Georgia is reimbursed directly by the State of Georgia.

 

The program operates under a Home and Community-Based Waiver (1915c) granted by the Centers for Medicare and Medicaid Services.

Who is Eligible for Georgia ICWP?

To be considered eligible in Georgia, a person must be 21-64 years of age with severe physical disabilities and/or traumatic brain injury and requires a level of care that is typically provided in a hospital or nursing home. You can apply while residing in a facility or if you are at risk of being institutionalized if you do not obtain the appropriate care in your home or community.

 

Applicants for ICWP must meet the following criteria:

  • Be capable of directing their own services (individuals with a TBI do not have to meet this criteria)

  • Have a severe physical impairment and/or TBI that substantially limits one or more activities of daily living and requires the assistance of another individual

  • Be medically stable but at risk of placement in a hospital or nursing facility because community-based support services are not available

  • Be able to be placed safely in a home or community setting

Caregiver Support Program (CSP)
The Department of Veteran Affairs (VA) Caregiver Support Program (CSP) offers clinical services to caregivers of eligible and covered Veterans enrolled in the VA health care system. The program’s mission is to promote the health and well-being of family caregivers who care for our Nation’s Veterans, through education, resources, support, and services. There is a CSP team CSP Team Locator located at every VA facilities .
 
CSP carries out its mission through two programs:
   - Program of General Caregiver Support Services (PGCSS)
   - Program of Comprehensive Assistance for Family Caregivers (PCAFC)

Program of General Caregiver Support Services (PGCSS)
Services:

  • Skills training, mobile support, one-on-one coaching, group support and coaching, self-care, peer support mentoring, telephone support, online programs, and referrals to available VA and community resources.

Eligibility:

  • PGCSS is available to caregivers of Veterans enrolled in VA health care who served in any era.

Veteran Requirements for Caregiver Program
- Enrolling in VA Health Care: A Veteran must be enrolled in VA health care, or is a Service member on active duty undergoing a medical discharge.  Veterans can apply for VA health care benefits by completing and following submission instructions here  (VA Form 10-10EZ).

Questions about Caregiver Support Program

What is the New Option Waiver Program 
The New Options Waiver (NOW) and the Comprehensive Supports Waiver Program (COMP) offer home- and community-based services for people with a developmental or intellectual disability. A diagnosis of developmental disability includes mental retardation or other closely-related conditions such as cerebral palsy, epilepsy, autism or neurological problems. These are developmental disabilities that require a level of care provided in an intermediate care facility for people who have mental retardation. There are a little over 12,000 people with developmental disabilities served by the NOW/COMP programs in Georgia.

 
Purpose of the NOW/COMP Waiver Programs
The NOW waiver program enables individuals with less intense and urgent needs than out-of-home residential treatment or extensive waiver services to live independently in the community.
Goals for participants in the NOW program include:

  • Increasing the independence and quality of life for people with developmental disabilities

  • Supporting families in caring for relatives with developmental disabilities

  • Increasing the flexibility of service planning and delivery to meet an individual’s needs

  • Avoiding the need for more intensive services

 

The COMP waiver program

Is for individuals who need comprehensive and intensive services to live in the community. Individuals eligible for the COMP waiver program need out-of-home residential support and supervision or intensive in-home services to remain in the community.
